Position Overview/Description
Under the direction of the Program Medical Director, the Team Hospitalist is responsible for the medical management of patients identified as being under the care of the Hospitalist program.
Clinical Responsibilities:
Primary responsibility for the high quality and cost-effective medical management of patients identified as being under the care of the Hospitalist program. Duties include, but are not limited to:
- Admitting medical and surgical patients 16 years of age and older (Adult), but excluding obstetrical.
- Providing internal medicine or critical care services (to the extent Hospitalist is privileged) to patients
- Providing medical coordination and inpatient utilization management for surgical patients.
- Coordinating with patient’s PCP, hospital, or health plan whichever is applicable, all ancillary services related to inpatient episode.
- Coordinating with patient’s PCP, hospital, or health plan, whichever is applicable, the discharge planning needs with the patient, patient’s family and the care manager/discharge planner
- Providing appropriate communication to PCP in order to insure continuity of care and appropriate follow-up care, including preparation of the discharge summary.
- Coordinating the transfer of patients to other hospital as required and medically appropriate
- Responsible for responding in timely manner to a contact from Cogent Call Center, hospital, regarding the care of patient under care of Hospitalist program.
- Responsible for the clinical/administrative oversight of the Clinical Care Coordinator.
Minimum Qualifications
- MD/DO, BC or BE in Internal Medicine or Family Practice (Critical Care or Pulmonary Certification a plus)
- Excellent analytical and problem solving skills
- Demonstrated persuasive clinical leadership
- Excellent communicator, both written and verbal
- Ability to work as member of team
- Fundamental knowledge of PC based applications
