• Anticipated weekly work schedule at base location: 8-5 M-F          
• Anticipated volume of office patients per day at base location:: 10-15
• Please note past volumes and projected volumes: 5-10  
• Anticipated volume of procedures per day at base location: 0-2
• Call description weekdays and weekends: Weekend rotation of 1:13
• Average inpatient practice volume per week or month: 5-10/week
