Seeking
1 BE/BC Internal Medicine or Family Practice Geriatrician Physician
- Must be BE/BC in Internal Medicine or Family Medicine with add'l certification or Geriatric Fellowship trained.
- Join an established and successful geriatric program with a department of 5 geriatricians and 20+ geriatric nurse practitioners.
- Work in collaboration with geriatric nurse practitioners to assess and manage the medical care of the elderly in skilled nursing facilities and transitional care settings.
- Full time.
- Telephonic call only-every 5th weekend and weekday night.
- No hospital care involved.
- Electronic Medical Records system being implemented in March, 2010.

Compensation: - Salary Range Market Competitive.

Community Description: Health care services for patients aren't limited to a clinic or hospital setting. SeniorCare Transitions helps patients who need medical care in transitional care units (short-term care facilities) and nursing homes (long-term care facilities). We work with patients and their families to provide routine care, manage medical problems, monitor rehabilitation and plan for discharge.
