Nurse Practitioner- Diabetes
Practice Location: West Bend, WI- 25 miles NW of Milwaukee
The Nurse Practitioner of the West Bend Clinic Endocrine and Diabetes practice will be responsible for providing patient care and education for Diabetes and Primary Care patients in collaboration with our 70 + primary care and specialty physicians. This includes health assessments, extensive diabetes education, and medication management. The NP will also provide evaluation of diabetes self-care, education and management to patients. The Nurse Practitioner should be experienced and familiar with specialized Diabetes and Primary Care and be comfortable making adjustments for insulin, insulin pumps and making primary care decisions.
The main responsibilities of the diabetes nurse practitioner will include daily interactions with diabetes patients in an office setting which integrates the national standards for diabetes. The diabetes NP will collaborate with the patients’ primary care physician to improve diabetes patient outcomes by using the Institute of Medicine concepts of patient centered care and decrease variation in diabetes care by using evidence based medicine-protocols. The diabetes NP will also be responsible for the continuous development of the diabetes education programs within the clinic plus community and participate in the collection, analysis and the impact of data related to patient outcomes.
The NP will work within the Endocrine and Diabetes practice which includes an Endocrinologist, 1 Nurse Practitioner and 2 RN Diabetes Educators; we are adding a second NP to the practice due to continued growth.
This position will be an employed hourly position working 4/5 days per week (possible 3 day a week work schedule in the future).
