Siouxland Community Health Center (SCHC) serves the tri-state area where Iowa, Nebraska, and South Dakota meet, an area dubbed "Siouxland." The 50-mile service radius is largely comprised of the Sioux City metropolitan statistical area with an estimated population of 141,742 but is surrounded by several rural communities.

Siouxland Community Health Center was founded in 1991 as a collaborative effort among two local hospitals, St. Luke's Regional Medical Center and Mercy Medical Center (then Marian Health Center). This effort was in response to a community need to increase access for health centers. In 1992, the health center saw its first patient as a small medical clinic with 12.

Today, SCHC is a Federally Qualified Health Center (FQHC) that provides multiple services and employs a workforce of 134 staff including 5 physicians, 6 FP physician assistants/nurse practitioners.  Services include primary medical care, dental services, short-term mental health counseling and other social services, and onsite pharmacy, laboratory, and radiology services. The health center also provides comprehensive care for HIV/AIDS, the only comprehensive provider within a 100 mile radius of the service area.

In 2009, construction was completed on a new 61,000 square foot health center, a project that was determined the best response to continued growth. In 2008, SCHC served 16,172 total patients in 66,186 visits, a 15% increase in patients and 5% increase in visits from the prior year. The new facility, which will support long-term growth, is also supportive the SCHC mission to provide primary healthcare that eliminates access barriers and improves the health of the Siouxland community.

Call coverage mostly outpatient call responsibility.
