Jordan Valley Community Health Center is a Federally Qualified Health Center designated to serve the uninsured and underinsured of Southwest Missouri. It is our mission and goal to become a medical home for those without access to primary healthcare.
A federally qualified health center (or “FQHC”) is an organization designated by the U.S. Department of Health and Human Services to provide healthcare services. As an FQHC, access to healthcare at Jordan Valley Community Health Center is improved through a sliding fee program. The sliding fee discount schedule is based on the poverty guidelines and allows qualifying patients with access to services at a reduced rate. In addition to the sliding fee program, services are also provided to patients with Medicaid, Medicare and commercial insurance. Jordan Valley Community Health Center provides the highest quality of care to all patients regardless of the payor source.
