Partnership HealthPlan of California (PHC) is a non-profit community based health care organization that contracts with the State to administer Medi-Cal benefits through local care providers to ensure Medi-Cal recipients have access to comprehensive, quality health care. PHC provides care to over 514,000 members. Beginning in Solano County in 1994, PHC now provides services to 14 Northern California counties - Del Norte, Humboldt, Lake, Lassen, Marin, Mendocino, Modoc, Napa, Shasta, Siskiyou, Solano, Sonoma, Trinity, and Yolo.
PHC delivers care to a large geographically diverse population. The 14 counties we serve cover over 38,000 square miles, which is larger than Connecticut, Delaware, New Hampshire, New Jersey, Rhode Island, and Vermont combined. Geographically speaking, we are the largest Managed Care Plan in California.
There is no better place to practice medicine than in rural Northern California. You can be minutes from the ocean, world-class skiing on the slopes of the Sierra Nevada, or from fishing in many of the crystal clear rivers and streams. The smaller communities remove you from the noise and the hustle and bustle of larger cities and will quickly welcome you in as part of the family.
PHC recognizes that practicing in rural commitment presents many challenges, but we are committed to being an active partner with physicians and provide as many resources’ as possible. These resources include grant funding to improve access, quality financial incentives, recruitment, and much more.
Another way we are able to overcome these challenges is by placing regional offices strategically throughout the 14 counties. We currently have four regional offices that is staffed by individuals from the community who have an intimate knowledge of the community’s needs. These offices are a great resource to physicians in neighboring communities.
Additionally, PHC staffs a robust Quality and Performance Improvement Department. This department works closely with providers and physicians to improve access and increase quality of care. A few of the programs administered through the Quality and Performance Improvement Department includes:
- Quality Improvement Program – This program works alongside providers to offer technical assistance through training on specific topics that includes, childhood immunizations, advanced care planning, best practices and more. To aid physicians and facilities who provide quality care, PHC awards quality financial incentives. Last year alone PHC awarded more than $24 million.
- Coleman - The PHC Coleman DPI Collaborative is an opportunity for contracted PHC Primary Care Providers (PCPs) to improve appointment access, decrease no-show rate, reduce cycle time, and use team-based care and operational data to achieve true transformation
- HEDIS - PHC uses the HEDIS tool to identify areas where we can focus our improvement efforts
- Partnership Improvement Academy - The Academy provides a space for clinicians, administrators and staff to gain quality improvement expertise from industry leaders and peers.
