Provida Family Medicine
Provida Family Medicine was founded by Dr. Luis I. Salazar in 1992. Dr. Salazar, mid-level providers, clinical and support staff offer a wide range of medical services to a diverse patient population that includes newborn children through the elderly. We do not offer obstetrical services. The bilingual skills of the practitioners and staff attract a large Spanish-speaking community of patients.
Lake County has experienced a substantial amount of growth in the past 20 years and has subsequently seen an influx of medical facilities and physicians. Provida Family Medicine has maintained a competitive advantage that will project into the future due to its wide range of services, participation in insurance and employer health plans, Spanish speaking skills and recognition as a Level 3 Patient Centered Medical Home by NCQA.
Electronic medical records were implemented in 1999 shifting to a paperless medical record system well before the healthcare market demanded the change. A computer in each examination and at each workstation allows for patient charts to be updated in real time.
Accuracy of medical record-keeping and follow-up are key components of our operations as well as a necessity in today’s medical environment. Our EMR data also service us on the measuring of clinical outcomes and in meeting Quality Initiatives for our affiliate institutions.
The medical practice conducts all billing functions inhouse, electronically.
Provida Family Medicine has expanded its facilities and staff in order to strategically position itself to handle and profit from the increased emphasis placed on Family Medicine and the burgeoning Lake County population. We currently care for 10,000+ lives and expect continued growth in the next several years.
