We are proud to be an independent multispecialty medical group in the Capital Region, NY - and we are truly independent. Although we pride ourselves on our relationships with the many hospitals and health plans in the region, we are owned and governed by our physicians; physicians of multiple specialties who are all practicing medicine and have a pulse on the front lines of healthcare.

CCP’s business model is comprised of decentralized clinical and operational decision-making at the practices level supported by a central administrative infrastructure working in partnership with the practices to relieve the physicians of administrative burdens and to gain economies of scale. The highest priority of everyone at CCP is to deliver top quality and efficient health care to our patients and doing so in a manner that promotes professional satisfaction with our physicians and staff.

CCP is built on relationships – relationships with our patients, our colleagues, and our communities. Internal Medicine, and primary care, in general, are the foundation of healthcare and CCP. We have over 40 primary care practices that serve as important portals of entry into our system when most of our patients have their first CCP experience and begin building lifelong relationships.
