Community Health Care, Inc. provides the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.

For 40 years, CHC has offered preventive, acute and chronic care, as well as laboratory, pharmacy, nutrition, and health education services more than 40,000 patients in the Quad Cities Area.  We are accredited by TJC and have achieved Level 3 Patient Centered Medical Home status.

We are proud of our medical team which includes 13 Physicians and 12 NP's and PA-C's. 

CHC is located in the center of the Midwest within 300 miles of seven major metropolitan areas: Chicago, Milwaukee, Des Moines, Minneapolis, Omaha and St. Louis. We are located along the banks of the Mississippi River and we serve two states and five cities. With a combined population in excess of 375,000, the extended community offers the energy of a big city with all the hospitality of a small town. There are numerous small towns that surround the Quad Cities that are also served by our organization.
