Harbor Health Elder Service Plan - Delivering Independence for 22 Years
For the last 22 years, Harbor Health Elder Service Plan has delivered a Program of All Inclusive Care for the Elderly (PACE) in the Greater Boston and recently Southeastern Massachusetts areas. A program of comprehensive, customized healthcare services developed to meet the needs of older adults with chronic medical conditions and their caregivers. Our mission is to help keep older adults healthy while experiencing personal independence living in their own homes and communities.
The PACE model of healthcare not only provides participants the care they require, but supports their caregivers as well.
Services can include primary medical care, home health services, behavioral health, rehabilitation programs, transportation, medications, hospitalizations, dietician support and meal programs, durable medical equipment and even family caregiver support. This comprehensive service package is provided at the PACE site in conjunction with Adult Day Health Services and when needed, in the home.
The Care Team develops a customized Care Plan to meet each participant’s needs. The Care Plan is developed in union with the participant and their caregivers. A team of physicians, nurse practitioners, registered nurses, social workers, dieticians, physical and occupational therapists, pharmacists and other health care professionals coordinate the Care Plan. The team meets regularly with the participant and their caregivers to adapt the plan to meet any changing needs.
With the addition of our second PACE location in Brockton, our service area now includes 18 cities and towns in Plymouth and Bristol County.
