Family Practice & Counseling Network

Family Practice and Counseling Network, a program of Resources for Human Development, is a network of community health centers providing comprehensive primary care, dental and behavioral health services. FPCN consists of three comprehensive health centers in Philadelphia – Abbottsford-Falls Family Practice & Counseling in the Northwest, the Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University in the North, and the Health Annex in the Southwest; a convenient care clinic in Philadelphia – QCare; one health center in York, PA.—East Side Health Center; and a Philadelphia site focused on behavioral health services to children and their families– Rochelle Center for Children’s Clinical Services. Today the Network serves more than 20,000 patients annually and is an NCQA certified Patient-Centered Medical Home.
 
The Network employs a nurse-managed health center model to deliver caring, quality primary health care, behavioral health services, dental care, and prevention programs that integrate services in treating the whole patient. Primary Care at the Network is provided by Nurse Practitioners with advanced practice degrees and the expertise to diagnose illness and prescribe medication, to make referrals to specialists, to provide pre-and post-natal care, and to offer a wide variety of other primary health care services.
