Community Health Care, Inc. provides the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.

For more than 40 years, CHC has offered preventive, acute and chronic care, as well as laboratory, pharmacy, chiropractic, nutrition, and health education services to more than 40,000 patients in the Quad Cities Area.  We are accredited by TJC and have achieved Level 3 Patient Centered Medical Home status.. 

CHC is located in the center of the Midwest.  We are only 130 miles from Chicago and within 300 miles of six other major metropolitan areas: Milwaukee, Des Moines, Minneapolis, Omaha and St. Louis. We are located along the banks of the Mississippi River and we serve two both Iowa and Illinois.  With a combined population in excess of 400,000, the extended community offers the energy of a big city with all the hospitality of a small town. There are numerous small towns that surround the Quad Cities that are also served by our organization.
