Yakima Valley Memorial (YVM) is seeking a Hospice and Palliative Care Physician. Our Hospice and Palliative Care Physician role includes: developing and carrying out patient management plans within the context of the interdisciplinary team, providing effective counseling and education to patients and their families to facilitate patient-focused and value driven healthcare and promoting informed decisions about diagnostic and therapeutic interventions based on patient information and preferences. This position requires inpatient consultation, hospice admissions, in-home hospice visits, palliative care clinic visits and inpatient hospice center rounding at Cottage in the Meadow, YVM’s hospice facility that provides a home-like environment and a 24-hour nursing staff. Our program includes home health assistance, palliative care, in-home and inpatient hospice, chaplain services and bereavement services.
