Hospice Nurse Practitioner Job with Northeast Georgia Health System in Gainesville, GA

NGPG is currently seeking a Nurse Practitioner to join our Hospice team in providing exceptional, compassionate care to our growing community.

This role involves providing comprehensive hospice and in-home palliative care service needs across a variety of settings including, home visits, skilled nursing and long-term care facilities. This is a highly autonomous and flexible role where you will have the ability to choose your own schedule while managing your own patient panel. You will have access to an extensive support and resource network including an interdisciplinary team comprised of a Medical Director, social worker, chaplain. Opportunity Overview:

• Hospice and in-home palliative care program provides patient-centered care in a supportive environment to patients and families facing advanced illness and end-of-life challenges providing in-home management of patients, leading discussions about serious illness and end-of-life, goals of care, and advance care planning while providing complex symptom management and complete hospice recertification visits.  Medical Director will provide mentorship and backup.
• The majority of our patients are served where they prefer to be at end of life to maximize quality of life – their home.  NP will independently provide palliative care and hospice face to face visits in patient's location including in-home, skilled nursing facility, and hospital as well as telehealth.
• Collaborate closely with a vibrant interdisciplinary team consisting of Social Work, Spiritual Care Services, Nurse Practitioners and Physicians and hospice and homecare teams.   Excellent  work culture & collegiality among providers and staff.
• Assess and manage symptoms of patients with advanced illnesses. Recommend treatment as appropriate in a consultative model with the primary care providers and other disciplines involved in the patient’s care.
• This position is full time, Monday through Friday. Schedule offers flexibility providing great work/life balance. Mileage reimbursement, phone and laptop provided.

Position Highlights:

• 5-6 patient visits per Provider daily
• Call coverage: 1 weekend per month, no night call and holiday on rotation
• This position requires daily travel (mileage reimbursed). 
• EMR Platform - Epic

Education, Training, & Experience

• FNP or AGACNP
• Ability to obtain an active license in the state of Georgia
• Functions well in a team environment, contributing to and supporting organizational goals and initiatives.
