Family Medicine Nurse Practitioner Job with SSM Health in Waupun, WI

SSM Health Wisconsin is actively recruiting a full-time (0.8 FTE) Family Medicine Advanced Practitioner based in Waupun, Wisconsin. This position will support the Waupun Wellness Clinic which offers care to the Waupun school district employees and their families. This position requires the flexibility and the ability to work independently. Hours for this position are TBD, but will be 24 hours per week M-F. There are no holiday or weekend hours. This facility will offer scheduled appointments and walk-in care, will provide CLIA waved lab tests, and also offers the opportunity to provide health risk assessments, wellness education, and diabetes education classes. Qualified candidates will have a minimum of three years' experience working as a physician assistant or nurse practitioner in primary care, urgent care, walk-in care, or in an emergency department.

The addition FTE will float to other DTE clinics.
