What is PACE?
A Program of All-Inclusive Care for the Elderly, which is designed for frail, 55+ adults, who meet criteria for nursing facility placement, but prefer to live at home and have medical care and supportive services provided. 

Our PACE program
We typically bus our patients in to receive care like rehabilitation and check ups, but also provide social events, games and a sense of community. Our PACE program in one of 11 in the state of Virginia and one of 100+ nationwide.

A minimum of one year experience working with the elderly population, chronic disease management, long-term care or disabilities.

Requires: MD or DO from an accredited medical school, and board certification in Internal or Family Medicine; Board certification in Geriatric Medicine is preferred.
