Location: Fort Hood, TX

The contract HCPs shall be a Neurology Physician who is competent, qualified and adequately trained to perform
assigned duties. The contract Neurologist performing under this contract shall perform services under the control
and general supervision of the Chief of the applicable department or his/her designated representative. The
contractor shall meet the following minimum qualifications.

1.2.1. Be a Doctor of Medicine (M.D.) or Osteopathy (D.O.) who has completed an internship and residency in an
accredited Neurology Training Program and is board certified or board eligible in Neurology. Must meet all
licensing/certification requirements to perform as a Neurology Physician. Must have and maintain a valid,
unrestricted license in one of the 50 states in the U.S., the District of Columbia, the Commonwealth of Puerto Rico,
Guam, or the U.S. Virgin Islands. Foreign medical graduate physicians shall possess a valid certificate from the Educational Council for
Foreign Medical Graduates.

1.2.2. Experience. The Neurologist shall have a minimum of two years of recent experience having been continuously
employed in the practice of Neurology or in training in the United States for the two years prior to commencement
of services under this contract. Have sufficient experience to be able to adequately diagnose and treat diseases and injuries to include, but
not limited to the following:
Peripheral Neuropathy
Multiple Sclerosis
Parkinson’s Disease
Traumatic Brain Injury Have sufficient experience to be able to adequately perform the following medical procedures, including,
but not limited to the following:
Electroencephalograms (EEGs)
Evoked Potentials

1.2.3. Contract Neurologist must possess a valid Basic Cardiac Life Support (BCLS) certification during the
contract period of performance. Certification shall be renewed biannually. Certification will be done by the
American Heart Association.

1.2.4. HCP shall be able to read, write, and speak English fluently to effectively communicate with patients and
other HCPs.

1.2.5. Contract HCPs shall be required to meet Continuing Medical Education (CME) requirements. The
government will not reimburse for these costs.

1.2.6. The contract HCPs have received Occupational Safety and Health Administration (OSHA) required training
for the position in which they will perform, and that training currency is maintained. Documentation of such
training shall be provided to the COR.

1.2.7. The positions require access to DoD computer systems; therefore, each Neurologist shall be a United States
citizen, or otherwise meet the requirements of Army Regulation (AR) 25-2.
